It is one of the most studied and widely used approaches for spine-related pain — but it is also frequently misunderstood. As a therapist certified in this method (Cert. MDT), I want to explain what it actually is and why the certification matters for your care.

What the McKenzie Method Actually Is

The McKenzie Method, formally known as Mechanical Diagnosis and Therapy (MDT), is a systematic approach to assessing and treating back, neck, and extremity pain. Rather than focusing only on where it hurts, MDT looks at how your symptoms respond to specific, repeated movements.

The key concept is directional preference: most mechanical spine problems have a particular movement direction that reduces, abolishes, or centralizes the pain. "Centralization" means pain that has been radiating into your leg or arm begins to retreat back toward the spine — a strong sign that we are on the right track.

Why Certification Matters

Many clinics list "McKenzie" as a technique, but a true Cert. MDT credential requires rigorous post-graduate training and a formal examination. It is not a weekend course.
